At the Department of Biology you must report sick to Lone Nørgaard Bruun, lyn@biology.sdu.dk, on the first day of illness, no later than the start of your normal working day.
If you are to undergo surgery or prolonged treatment, you must inform your immediate manager of the dates of your absence as soon as you know them.
The filled in and signed form must be handed in to Lone on the first day after absence.
